This is a companion report to Report 2014s, Safety performance indicators - 2014 data. It gives details of reported events which have resulted in a fatality. It includes descriptions of the incidents, and suggested corrective actions to prevent recurrence as well as categorising the incidents by work function, activity, and the corresponding Life-Saving Rule (see Report 459).
