<p>Continuing the initiative of IOGP’s Safety Committee to feed back learnings from events, and to help organizations to categorize process safety events (PSE), the fatal incident and high potential event descriptions from the 2014 IOGP safety performance database that are process safety-related, and those that are also process safety events, have been selected for separate publication.</p> <p>This compilation of incident and event descriptions is published as IOGP Report 2014pfh. The complete set of fatal incidents and high potential events has previously been published as IOGP reports 2014sf and 2014sh.</p>
