<p>This is a companion report to IOGP Report 2015s, <i>Safety performance indicators - 2015 data</i>. It gives details of reported events which have resulted in a fatality. It includes descriptions of the incidents, and suggested corrective actions to prevent recurrence as well as categorizing the incidents by work function, activity, and the corresponding Life-Saving Rule (see <a href="http://www.iogp.org/pubs/459.pdf">IOGP Report 459</a>).</p>
