<p>This is a companion report to IOGP Report 2016s, <i>Safety performance indicators - 2016 data</i>. It gives details of the reported incidents which have resulted in a fatality. It includes a narrative description for each incident and suggested corrective actions to prevent recurrence. Incidents are categorized by work function, activity, and the corresponding Life-Saving Rule (<a href="http://www.iogp.org/bookstore/product/life-saving-rules/">see IOGP Report 459</a>).</p>
